DBE/MBE/WBE Compliance for all Department of
Aviation Contracts The reporting of DBE/MBE/WBE compliance for all Department of Aviation contracts will be accomplished by using the Department of Aviation Quarterly DBE/MBE/WBE Utilization Report. This form must be submitted to the Department of Aviation Procurement/Contracts Administration Unit by October 6, 2006, and thereafter will be due on or before the 5th business day following each calendar quarter. (Calendar quarter ends September 30, 2006, report is due October 6, 2006.) The Department of Aviation Quarterly DBE/MBE/WBE Utilization Report should reflect the aggregation of all subcontractors used on the contract, as well as all invoices sent to the City of Chicago for the previous quarter. The form requires the listing of all invoice numbers and the total amount billed to the City for the preceding quarter.
